Norfolk First Support

The Norfolk First Support service provides short-term support in your home for up to 6 weeks, usually after you've been in hospital or you’ve experienced a change of circumstances, for example due to an illness. This is a free service for up to 6 weeks.

A practitioner will assess you and decide if you could benefit from support from us.

If you are referred to Norfolk First Support, we'll carry out an initial assessment of your needs. We will then work with you to develop a personalised support plan and discuss with you what’s important to you. This is designed to help you regain the skills and confidence to remain as independent as possible in your home. 

Who can get help?

Anyone from the below groups who is over the age of 18 and assessed as eligible:

  • People with physical illness
  • People with learning disabilities
  • Carers

What happens at the end of the reablement support?

Many people will not need support for long and will be able to return to being independent quickly. If you still need help after your reablement has finished, or it becomes clear at any point that you will need ongoing support, then we will discuss with you other options that would help you to stay in your home. This could include a care package or other support, for example assistive technology, or helping you to get involved with your community again.

What will happen when the Norfolk First Support team arrives?

The Norfolk First Support team will always:

  • Show their identity cards when they visit you for the first time
  • Be dressed in the team uniform
  • Behave professionally and politely
  • Respect your privacy and dignity
  • Ensure you receive prompt, appropriate support and notify you if there is a delay
  • With your agreement and if appropriate, make ongoing referrals to other support
