NHS Choices Continuing Healthcare

NHS Continuing Healthcare (sometimes called fully-funded NHS care) is free care outside of hospital that is arranged and funded by the NHS. It is only available for people who need ongoing healthcare and meet the eligibility criteria. You can also ask for an assessment for the person you look after by talking to a health or social care professional working with them or the NHS continuing healthcare coordinator at the primary care trust. You can find out the name of your local co-ordinator by asking your GP, contacting your local Patient Advice and Liaison Service (PALS), or contacting your Clinical Commissioning Group directly.
