Falls Prevention Service

The Falls Prevention Service, provided by Norfolk Community Health and Care NHS Trust, is for people who are 65 years and over who have had two falls in the last six months or a fall that has resulted in a hospital admission or injury.

Under 65's can be referred to therapy/nursing for general assessment. The team provides home-based assessment to identify the factors and risks which may contribute to a person’s falls and develop an action plan to improve their confidence, balance, strength and stamina in order to reduce their risk of falling in the future.
